New keepsake format added

I have added a new suggested keepsake format to the appendices.  This is a simple sketchbook journalling layout which splits a page into 'blocks'.  The format is intended to make the blank page less daunting for players/learners who don't identify as 'artistic', but also means that space is limited for each memory, which will help to keep entries succinct.
